(TV) Insert for S/T LP promo?

Did the promo LP for the self-titled Television album come
with an inner sleeve /w lyrics?  I won a copy on eBay
recently and was told it included one, but when it arrived
in the mail today, the sleeve was for some band called
Warrior Soul.  I'm just wondering if a lyric sheet actually
exists or not...

An empty outer sleeve to Barbara Streisand's "Songbird"
album was also included.  I have no idea why.
